Graduates of the MSHS in Physician Assistant Program can visit the American Association of Physician Assistants for the state board of examiners and requirements for licensure across the United States. Please do not contact the program with questions related to specific state requirements.
Physical Therapy
Graduates of the Doctor of Physical Therapy Program can visit the Federations of State Boards of Physical Therapy for contact information for the state board of examiners and requirements for licensure across the United States. Please do not contact the program with questions related to specific state requirements unless you have already contacted the state board of examiners and need additional assistance.
Requesting Transcripts and Enrollment Verifications
Visit the Office of the Registrar’s (OTR) Transcripts & Certifications website to order copies of your transcript and request an enrollment or graduation verification/certification.

- Verifications: Use the Request for Certification Services webform to request a letter or have a form completed
- Letters OTR can provide: Enrollment for a given semester (available starting the first day of the semester), Registration (student must be registered at the time of request), Degree(s), Expected degree and date, Invitation Letter, Letter of Completion, Other
- Forms: if you have been provided with a form that must be completed by OTR, you may upload that form and include details on the specific request.
