If a patient is diagnosed elsewhere and treated at our facility, do we need to provide an SCP to comply with the CoC SCP standard?

Answer: 

Yes, if two separate facilities are providing treatment, both facilities must collaborate to complete and provide the comprehensive care summary. In all cases programs, hospitals, and physician offices should work together to provide the information necessary for completion of a plan.
